About UW Medical Center Anticoagulation Services
The anticoagulation services program at the University of Washington Medical Center is operated by the Department of Pharmacy. Services encompass the management of anticoagulant therapy in four pharmacist-managed anticoagulation clinics as well as coordination of the use of antithrombotic agents in the inpatient setting. Pharmacist providers are involved in clinical practice, training and education, and research activities consistent with the mission of the Medical Center and the Department of Pharmacy.

"The goals of pharmacist-managed anticoagulation services include treatment and prevention of thromboembolic disease and minimization of complications of antithrombotic therapy."
